This is why fibromyalgia doesn’t show up on X-rays or standard blood work: the problem is in how pain is processed, not in the joints or muscles themselves.

Is Fibromyalgia an Autoimmune Disease?

Not by current classification  fibromyalgia doesn’t cause the tissue inflammation typical of autoimmune diseases like rheumatoid arthritis or lupus, and there’s no confirmed autoantibody marker for it. Duloxetine in fibromyalgia works by rebalancing pain-related neurotransmitters in the brain and spinal cord, and is one of the most well-studied fibromyalgia medications. There’s no single “best medication for fibromyalgia”  the right choice depends on which symptoms (pain, sleep, mood, fatigue) are most disruptive for you, decided together with your doctor.

For fibromyalgia nerve pain medication specifically, pregabalin and gabapentin work by calming overactive nerve signalling, though gabapentin remains off-label for this use. Over-the-counter pain relievers (acetaminophen, ibuprofen, naproxen) may help mild pain, but opioids are not recommended  research hasn’t shown them to be effective for fibromyalgia long-term, and they carry real dependence risk.
